The Challenge
How do you communicate the value of team-based rehabilitation care through one person’s story?
The Dufferin Area Family Health Team (DAFHT), serving communities including Orangeville and Bolton, wanted to highlight the impact of its rehabilitation and chronic care support services through a more personal and human-centered approach.
The project focused on communicating the realities of living with arthritis while also demonstrating the role collaborative healthcare teams can play in improving quality of life for patients managing chronic conditions.

The Approach
One patient’s journey, anchored by clinical insight and natural conversation.
Motion Narrative developed a patient-focused documentary story centered on one individual's experience living with arthritis and working with DAFHT's rehabilitation team. Through thoughtful interviews with both the patient and healthcare providers, the project combined personal storytelling with clinical insight — helping translate the patient journey into a clear and emotionally grounded narrative.
The film emphasized authenticity, preparation, and natural conversation, allowing both the healthcare team and patient experience to be presented in a way that felt accessible and relatable.

The Impact
Coordinated care, told as lived experience.
The final piece helped communicate the value of team-based rehabilitation care through real patient experience, giving audiences a clearer understanding of how coordinated support services can positively impact long-term health and daily life.
By focusing on human outcomes and lived experience, the project supported broader communication efforts around chronic disease management, patient-centered care, and the role of integrated healthcare teams within the community.
