The Challenge

How do you communicate a coordinated-care initiative beyond policy language?

Ontario's Health Links program was developed to improve care coordination for patients with complex healthcare needs — bringing together hospitals, family physicians, long-term care providers, and community organizations through a more integrated model of care.

The initiative involved complex healthcare systems and collaborative delivery models that could be difficult to communicate clearly to both decision makers and the broader community.

The Approach

A series of patient-focused stories grounded in real experience.

Motion Narrative produced a series of patient-focused documentary stories designed to translate the goals and impact of the program into clear, human-centered narratives.

Through thoughtful interviews and real patient experiences, the project highlighted how coordinated care teams were working together to deliver more personalized and connected healthcare for patients and their families.
